Excellus BlueCross BlueShield works with PopHealthCare, LLC ("PopHealthCare") an independent company with an affiliated medical group, to offer the "CareSight" program to eligible Medicare Advantage members. The CareSight program can help members manage chronic conditions efficiently and effectively.
As part of the CareSight program, PopHealthCare uses a care team of nurse practitioners who provide home visits and registered nurses who provide telephonic support and care coordination. The CareSight program can help members manage chronic conditions to avoid problems and identify changes in health before they become problems. The CareSight program does not replace a member's primary care physician or any of the member's specialists. Rather, it supports and complements care for a high risk Medicare Advantage member with multiple medical, functional, psychosocial and behavioral needs. The CareSight program is available 24 hours a day, 7 days a week.

Excellus BlueCross BlueShield contracts with the Federal Government and is an HMO plan and PPO plan with a Medicare contract. Enrollment in Excellus BlueCross BlueShield depends on contract renewal.
