To be eligible for enrollment, you must have both Medicare Part A and Part B and reside in our service area
- Central New York Region: Broome, Cayuga, Chemung, Chenango, Cortland, Herkimer, Jefferson, Lewis, Madison, Oneida, Onondaga, Oswego, Schuyler, Steuben, St. Lawrence, Tioga, Tompkins counties.
- East Region: Clinton, Delaware, Essex, Franklin, Fulton, Hamilton, Montgomery and Otsego counties.
- Rochester Region: Livingston, Monroe, Ontario, Seneca, Wayne and Yates counties.
- Utica Rome Region: Herkimer, Madison and Oneida counties.
Excellus BlueCross BlueShield is an HMO plan and PPO plan with a Medicare contract. Enrollment in Excellus BlueCross BlueShield depends on contract renewal.
